Community Supported Living (CSL) Direct Care Professional is to provide direct support to individuals with disabilities to live independently within the community.

QUALIFICATIONS

REQUIRED

  • Must be 18 years of age or older
  • Access to reliable non-public transportation with required insurance coverage
  • Flexibility in scheduling
  • Ability to operate computer equipment; knowledge of various software packages including Office Windows applications 2003 and above

PREFERRED

  • Related two or four-year degree
  • Demonstrate basic knowledge and understanding of issues that affect people with disabilities
  • Previous direct or related experience

 

ESSENTIAL JOB FUNCTIONS

  1. Deliver quality community living services.
    • Activities include assisting with the development and implementation of development plan goals; support the participant in community integration skills (cleaning, cooking, menu planning, personal shopping, laundry, manage personal finances, appropriate dress, social interaction, recreation, community awareness, major grocery shopping); problem solves; educate participants in personal safety issues, and administer CPR/first aid when applicable.
  2. Assist participants in building natural supports to become integrated into their community.
    • Activities include assisting in the identification and development of recreation interests, foster the development of friendships in their neighborhood, connect the participant with people of similar interests, and provide education to the community.
  3. Monitor participant’s general health.
    • Activities include supporting the individual to follow physician’s recommendations, assist the participant in ordering medications and/or medical supplies; accompany the participant to various appointments (i.e. medical, hair, dental), advise participants regarding personal hygiene issues, and report any concerns to supervisor and other team members in a timely manner.
  4. Monitor and provide personal care to participants.
    • Activities include assisting participants with the use of the toilet, hand washing, personal hygiene issues, and prompting participants in skills of daily living.
  5. Serve as an advocate and assist individuals in understanding options and in communicating their choices.
    • Activities include inform participants of activities and events; act as an appropriate role model; maintain professional relationships with family, county staff, and Aptiv team members, serve as the primary parent contact, notify stakeholders of pertinent information relating to the participant.
  6. Complete all required documentation in a timely manner.
    • Activities include daily/weekly/or monthly updates on participants, billing sheets, case notes, medical update form, and all other pertinent documentation to ensure the current case file.
  7. Maintain high-performance standards.
    • Activities include follow organizational procedures and policies; attend team meetings; maintain job knowledge through training and orientation; provide quality customer service; manage time; use resources wisely, and maintain solid working relationships, confidentiality, and ethical standards.
  8. Maintain current knowledge and assure compliance with applicable laws, rules, regulations, Aptiv policies and procedures, and national standards.
    • Activities include periodic review of Aptiv policies and procedures and national standards; maintain and practice all aspects of safety while performing duties.
  9. Must be capable of working a flexible schedule.
    • Shifts include weekdays, weekends, and holidays with the ability to transition between staff. Staff is expected to stay overnight at the live-by apartment for five nights each week unless the person has approved PLT. Unpaid sleep time of eight hours will be provided by staff. If sleep is interrupted to perform work duties on days unpaid sleep time is provided, staff will be paid for the time staff sleep is interrupted to perform such work duties. Staff is expected to carry and respond to the program on-call phone on scheduled workdays.

PHYSICAL/SENSORY/COGNITIVE REQUIREMENTS

The majority of the work time will be spent in the participant’s home or the community assisting the participants in achieving their development plan goals. Mobility within the rural and urban community is required on regular basis. Occasionally required to sit at a desk or in meetings for an extended period of time. Ability to present information and respond to questions from participants, staff, and the general public when speaking on the phone, corresponding on the computer, or in face-to-face interactions.

Uses hearing and vision with the ability to collaborate and orchestrate in a variety of environments. Regular use of fine motor skills for writing or typing. 

Lifting weights of up to 20 pounds regularly. Bending and walking are required regularly.

Regularly will concentrate on multiple tasks simultaneously along with the capacity to respond calmly and quickly to emergency situations required. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables. Capacity to differentiate between various levels of confidentiality and act accordingly.

TB test required before starting employment. Ability to obtain CPR/first aid and medication training certification, based on program requirements, within 30 days of hire.

ENVIRONMENTAL CONDITIONS

The majority of work is performed in the participant’s home or community setting with varied conditions. Outdoor travel is required daily. Staff is expected to work in an environment with participants who can be verbally and/or physically aggressive to staff, other participants, community members, and themselves. Aptiv provides training and support to prevent, de-escalate and manage these incidents, but cannot guarantee the complete absence of such occurrences.
